Car Travel Interruption Protection Reimbursement Form

Your claims must be submitted within (60) days of the loss. You can submit your request in one of two ways:

  • Online Submission — Complete the reimbursement request using the form below. As you fill out the form, refer to the helpful details and definitions provided throughout to guide you.
  • Mail-In Submission — If you prefer to submit your request by mail, click here to download and print the form.

These are the states we service: The Auto Club Group in Florida, Georgia, Illinois, Indiana, Iowa, Minnesota, Nebraska, North Dakota, Tennessee, Wisconsin and Puerto Rico; Auto Club Group Insurance Company in Michigan only.

Reimbursement requests for ROADSIDE ASSISTANCE (towing, battery service, etc.) require a different form. Click here to navigate to this form.
